STATEMENT OF FAITH AND TERMS OF SERVICE
Welcome to H.E.A.R.T.S. (Home Educators Applying Righteous Teaching Standards) for the 2023 -2024 School year! H.E.A.R.T.S. is a Christ support group formed to provide support, fellowship and encouragement to new and seasoned families in the home education journey. To achieve these goals, the group meets to pray for and encourage one another, share methods, curriculum, ideas, and to plan group events, field trips and classes.
We are a group of Bible-believing Christians and our group and activities reflect christian values. We believe The Bible is the only Word of God. We believe Jesus died to atone for our sins and He rose on the third day.
Agreement and Respect for these statements is necessary to join our group.

All H.E.A.R.T.S. Homeschoolers memberships for 2023– 2024 will expire 7/31/24

H.E.A.R.T.S. is run by its members. There are no paid employees. Our group is only as active and vibrant as our member families make it. As such, the group has a number of jobs that require an active member’s involvement! All Volunteer Members commit and help with at least one volunteer activity for H.E.A.R.T.S. during the membership period. Please take a few moments to review this list and consider where God would have you to serve in H.E.A.R.T.S. this school year. We will help you as much as you need and will pair you with several other members in each area. Volunteers make up our entire leadership structure and coordinate all of H.E.A.R.T.S. activities so we appreciate as much involvement as you are able to offer.
E-mail Updates: By completing the H.E.A.R.T.S. Homeschoolers membership application, you agree to receive regular email updates from H.E.A.R.T.S. for the duration of your membership. H.E.A.R.T.S. maintains this website specifically for its members. Some of the best information comes from members like you through our Members only Face book page or forums.

H.E.A.R.T.S. Homeschoolers Membership Rules of Behavior Agreement

God has provided us with a great place to hold some of our homeschool functions.
Our meeting location is Cooper City Church of God. This is not our mailing, Co-op, or contact address.

Please review the following “ground rules” when using the church building and grounds. It is very important that you explain these guidelines to your children so that we can provide the least amount of interruption to the office workers and staff as well as make sure our children are safe and supervised. We are very excited that the Lord has opened up the use of this facility.
1. We are to be in ONLY the rooms scheduled for use; if Children are present; stay in the designated areas at all times.
2. Let’s always leave the building and grounds looking better than we found them. God has truly blessed us with a great facility so let’s be good stewards of His blessings!
3. No weapons or anything that could be viewed as a weapon is to be brough into our meeting or any of our events. Failure to comply will result of termination of membership with no rights of renewal.3.

Rules of behavior for ALL H.E.A.R.T.S. Support Group Activities and Fieldtrips
1. No running, jumping, climbing where inappropriate.
2. No destruction of property.
3. No disrespectful or rowdy behavior.
4. No vulgar behavior or language.
5. Modest dress code required from parents and children
6. No dropping off children at ANY event without previously arranged adult supervision.
*We expect parental involvement at all events. It is our desire that all parents participate in all functions in which their children are participating. However, if that is not possible, it is the responsibility of the parent to arrange adult supervision.
AGREEMENT
I have read and accepted the terms and responsibilities for the Rules of Behavior Agreement We agree that our family will honor the rules of behavior at HEARTS Homeschoolers Support Group functions and that failure to do so may result in restriction from further activities.
“H.E.A.R.T.S. Homeschoolers” Support Group Statement of Health Insurance/Release of Liability
The undersigned parent or guardian of the above child/children releases Cooper City Church of God, its officers, and/or representatives as well as H.E.A.R.T.S. Homeschoolers Support Group, their Leadership Team, representatives and assignees from all liability for any and all injuries suffered or sustained at any time H.E.A.R.T.S. Homeschoolers is on the premises of Cooper City Church of God or during any field trip or event sponsored by the H.E.A.R.T.S. Homeschoolers.
The undersigned parent or guardian of the above child/children represents and warrants that my child/children has/have no physical, health related or other problems which would preclude or restrict his/her participation in any H.E.A.R.T.S. Homeschoolers activity or otherwise render his/her participation dangerous or harmful to them or others. I further represent and warrant that my child/children has/have adequate medical, health, and/or other insurance coverage for particiapation.
The undersigned parent or guardian of the above child/children agrees that my child/children must abide by all rules and regulatations applicable to participation in H.E.A.R.TS. Homeschoolers activities, at Cooper City Church of God, or elsewhere. Should my child/children require emergency medical treatment or fist aid as a result of illness or injury associated with participation in H.E.A.R.TS. Homeschoolers activities, at Cooper City Church of God, or elsewhere, I consent to such first aid and/or treatment and agree to pay for any and all related medical and hospital expenses associated with such treatment.
The undersigned parent or guardian of the above child/children understands Cooper City Church of God or HEARTS Homeschoolers are not responsible for medical expenses incurred as a result of an injury to the child/children while H.E.A.R.T.S. Homeschoolers is using the facilities of Cooper City Church of God or while participating in any field trip or event sponsored by H.E.A.R.T.S. Homeschoolers.
I have signed* this document in further consideration of my acceptance as a participant in H.E.A.R.T.S. Homeschoolers activities, Cooper City Church Of God, or elsewhere. I hereby, voluntarily assume any and all risk, including injury to my person or the person of the child for whom I sign as parent or guardian, and/or loss of property to said person or persons, which may be caused as a result of my or his/her presence at, or participation in, any activity.
I hereby, for myself, and as parent or guardian for my child or children, our heirs and personal representatives waive and release any and all rights and claims we may have with Cooper City Church of God, its officers and representatives or H.E.A.R.T.S. Homeschoolers, its Core group, representatives, and assignees for any and all injuries suffered by me or any person for whom I have signed as parent or guardian in connection with any participation in any H.E.A.R.T.S. Homeschoolers activity.
The undersigned parent or guardian further agrees to reimburse or make good any loss or damage cost that Cooper City Church of God, their officers, and/or representatives as well as H.E.A.R.T.S. Homeschoolers Support Group, their Core group, representatives and assignees, may have to pay if any litigation arises on account of any claim made by said minor/s or by anyone on behalf of said minor/s.
I CERTIFY THAT I AM 18 YEARS OF AGE OR OLDER AND THAT I HAVE READ AND FULLY UNDERSTAND THIS RELEASE AND INFORMED CONSENT FORM, AND I SIGN IT VOLUNTARILY WITH FULL KNOWLEDGE OF ITS SIGNIFICANCE.
